REQUIREMENTS FOR ADDITIONAL REGISTRATION OF THE P.G. MEDICAL QUALIFICATION.

The Applicant is required to submit the following documents in original with its 2 photocopies:

1. Original Permanent Registration Certificate of MBBS of the Chhattisgarh Medical Council.
2. Original recognized Post Graduate Degree / Diploma.
3. For registration fees & penalty amount, please see the prescribed fees chart.
4. The amount of fee, for obtaining any N.O.C. /Confirmation as fixed by the concerned university / institution, shall be payable by the applicant separately. as detailed in the fee statement.
5. Name of your Medical College with a proof of authentic document.

2. Photocopy of Registration Certificate

REQUIREMENTS FOR CHANGE IN SURNAME (ONLY FEMALE CASE)

1. An Application on plain paper addressing to the Registrar, Chhattisgarh Medical Council.
2. An Affidavit or Marriage Certificate
3. Original Registration Certificate (Without lamination)
4. The prescribed fee (see fee chart).

REQUIREMENTS FOR CHANGE IN ADDRESS

3. Photocopy of Registration Certificate.
